You usually must itemize deductions. Then you can deduct only the portion of your qualified medical expenses that is above 7.5% of your AGI under the IRS medical expense deduction rules. If you take the standard deduction instead, the fact that your premiums qualify as medical expenses may not reduce your taxes at all.

The IRS instructions for Form 7206 explain that self-employed individuals may be able to take a self-employed health insurance deduction, and those instructions also say that Medicare premiums you voluntarily pay to obtain insurance in your name can be used to figure that deduction, subject to the form’s limits and requirements. The IRS Form 7206 overview page says the form is used to determine any self-employed health insurance deduction that may be claimed on Schedule 1.

That does not mean every self-employed person can deduct the full amount. The business setup, earned income limits, and other IRS rules still matter.
